Community Health Workers for COVID Response and Resilient Communities (CCR)
The aim of this CDC funded CCR Program is to integrate CHWs into organizations and PHS to address high risk residents’ hesitancy to be tested and vaccinated for COVID-19, improve access to preventive health care services, increase management of underlying health conditions, and improve social determinants that serve as life barriers.
In partnership with 6 Community and Faith Based Organizations (Amelia Ann Adams Whole Life Center, Catholic Charities, El Concilio, Emergency Food Bank, Mary Magdalene Community Services, and STAND Affordable Housing), 14 CHWs throughout the county are working to:
- Reduce social and health inequities by connecting San Joaquin County residents to free resources and services.
- Build a sustainable Community Health Workers (CHWs) network in San Joaquin County.
- Address social determinants of health among San Joaquin County’s most vulnerable populations.
- Build partnerships and systems that access Medicaid reimbursements and other sources to sustainably support the CHW corps.
- Increase awareness of and access to available resources and services.
- Decrease social inequities.
