County Wide Coordination
Having healthy smiles will require collaboration and coordination among partner organizations, to be accomplished through the SJ Teeth Collaborative.
The SJ Teeth Collaborative meetings are held virtually quarterly, and membership is open to all interested parties. Contact sjteeth@sjcphs.org for more information.
Additionally, the sustainability of these oral health efforts, particularly care coordination, are essential for the ongoing improvement of oral health among San Joaquin residents.
The California Department of Health Care services offer care coordination support to Medi-Cal recipients.
